BT Test Providers

All physicians can order the BT Test one of two ways:

The physician/provider can write "BT Test" on their regular prescription pad and direct the patient back to this website to order the test.
- OR -

With the form here, the physician/provider only needs to sign the front page and have the patient fill out the form. Once one of the above steps is taken; the patient can then go to any of the draw locations to have her blood drawn for the test. Remember the patients must have a signature from her physician/provider in order to take the test.
