If you are interested in receiving the BT Test though your private physician, you can download the BT Test prescription and blood draw authorization form here. Take this form along with any other information you wish to download from our BT Information File (click here) to your personal physician/healthcare provider for his/her signature in order to get the test. The prescription form has a detailed list of the available blood draw Patient Service Centers and/or instruction on where you can go to get your blood drawn for the BT Test®. Of course, if your doctor’s offices has that capability in-house they can also draw your blood while you are at your appointment. The sample is then sent to Provista Diagnostics to be analyzed, after which the results will be sent to your physician for review

Provista accepts all private insurance company coverage. There is no co-pay for the BT Test®. Additionally, Provista will file all claims on behalf of the patient and accepts the insurance provider’s reimbursed amount as payment in full, subject to the routine and traditional patient deductibles for the individual policy. Provista recognizes that some patient may desire or need to pay cash for such testing. As such, for cash pay patients Provista accepts all major credit cards, and for individuals requiring financial assistance affordable payment plans are available.
