Ordering Options for the BT Test™
To order the BT Test you may choose one of the 3 ordering options below:
Option 1 – Using Provista Life Sciences Physician
If you are interested in receiving the BT Test directly though Provista Life Sciences, you can download the prescription form
here and then go to any of the laboratory locations on the prescription form for the blood draw. The sample is then sent to Provista Diagnostics, the laboratory arm of Provista Life Sciences, to be analyzed, after which, Dr. Corn will review the results. If the sample is returned abnormal, Dr. Corn will personally contact you to discuss the results.
Option 2 – Using Your Personal Physician
If you are interested in receiving the BT Test though your private physician, you can download the prescription form
here, go to your personal physician to sign the prescription form and then go to any of the laboratory locations on the prescription form for the blood draw. The sample is then sent to Provista Diagnostics to be analyzed, after which, the results will be sent to your physician for review.
Option 3 – Any Lab Test Now Scottsdale, AZ Location
You may also go directly to Any Lab Test Now to receive the BT Test. You can download the prescription form
here, fill it out and then go to the Any Lab Test Now Scottsdale location for the blood draw (8902 E. Via Linda, Suite 115, Scottsdale, AZ 85258, 480-451-0002). The sample is then sent to Provista Diagnostics. (This option is cash pay only at $295.)
Payment Options for the BT Test™
When ordering the BT Test, you and/or your provider will document certain medical history information either through the online process or through a paper system. Your healthcare provider will then prescribe the test on a BT Test/Draw Authorization Form, (or your healthcare provider can use their own prescription form). On the BT Test/Draw Authorization Form, you can choose a payment option by checking the appropriate box, Self Pay or Insurance.
Option 1 – Insurance Billing ($395)**
Provista Life Sciences through its laboratory services division Provista Diagnostics (PDx) is offering the BT Test ™ a blood test that assists in the early detection of breast cancer. PDx will file private insurance claims on behalf of women who take the test and in most instances cover 100 percent of the test’s cost. *(In some instances where a patient has an annual insurance deductible that must be met before insurance will pay for a prescribed service, the patient will be responsible for paying this amount).
If you choose Insurance Billing, you must enter your insurance information. PDx will bill the insurer for $395 for the BT Test. Testing will begin upon receipt of the blood sample, completed BT Test/Draw Authorization Form and complete insurance information. PDx will submit claims directly to insurance carriers and will appeal and resubmit claims if necessary.
** Medicare has not yet established a coverage policy for this test. Because Medicare will likely not cover test costs until a policy has been established, test costs must be paid by the patient, see Option 2.
Option 2 – Self Payment ($295)
If you choose to pay for the testing service yourself, testing will begin upon receipt of your blood sample, associated paperwork where you have checked Self Pay in Full and have provided your credit card information. You may also provide with your completed paperwork a check or money order payable to Provista Diagnostics, LLC for the amount of $295.
